Referrals and self-referrals to Early Intervention for ME in support of the healthy development of young children, birth-36 months, may be made by medical, clinical, and social services providers; childcare specialists; and parents, family members, friends, kinship, foster families, and other caregivers.

What will happen next?

Within three business days of receiving the referral, a representative from the Early Intervention for ME regional site serving the town in which the child resides will contact the parent/guardian. This representative will gather more information and discuss the concerns, provide information about early intervention, and determine the family’s interest in scheduling any screenings or evaluations to determine if the child is eligible for services. 

FAQ

What information is needed when making a referral?

The following information is required when referring a child to Early Intervention for ME:

  • the child’s name,
  • date of birth,
  • physical address,
  • with whom the child lives,
  • the area(s) of concern and any diagnosed conditions,
  • the parent(s) or guardian(s) name(s),
  • mailing address and phone numbers,
  • the referral source’s name and contact information.
  • If known, the child’s primary healthcare provider and any other agencies working with the child and/or family should be noted.
What happens after a referral?

After Early Intervention for ME receives a referral we can perform a free assessment of a child’s basic skills and eligibility for early intervention services. Once we understand a child’s unique developmental needs and the family’s priorities and goals, we form a team of dedicated early intervention professionals, led by a dedicated service coordinator who is with a family every step of the way.

Who will be on the early intervention team?

In addition to the Early Intervention for ME staff that make up a child’s Individualized Family Service Plan (IFSP) team, we collaborate with a wide range of professionals, such as experienced home health providers, physicians, specialists in hearing and vision, childcare centers, and community and human service agencies to provide coordinated and holistic services for each child.
